What to do if your email program does not accept the online application

Some email programs, like Lotus Notes, may not accept input from a form for transmittal.  Normally, the transmission is automatic, but in some cases, nothing happens or a new blank message is opened in the email program, without the data that had been input on the form.

In such cases, please do one of the following:

• Print a copy of the completed form and mail it to Christopher Gates, Underwriters Laboratories, 333 Pfingsten Road, Northbrook, IL 60062, 847.664.2618.
• Print the completed form to a pdf file and email it as an attachment to christopher.gates@us.ul.com
